NOTE – Following completion of the HMS Audit, the employer members of CCMHG contacted the employees that either did not respond to the audit or who submitted incomplete documentation. As a result of that effort, the final count of ineligible dependents and the return on investment may be lower than stated in the HMS audit report.
List of Required Verification Documents for Dependents – Employers should be asking for these documents when an employee enrolls or makes a change to add a dependent(s) to their health or dental plan.
